Declaration/ Undertaking/ Affidavit
To be produced once provisional admission is offered at SINHS
I, __________________________ S/o. ___________________________ selected for the Generic BSN Degree Program in the Sarhad Institute of Nursing and Health Sciences Mardan declare that:
1. I will abide by the rules and regulations of Sarhad Institute of Nursing and Health Sciences Mardan, as well as Khyber Medical University, enforced OR as amended from time to time, and I will not dispute them in any case.
2. I will abide by all the rules and regulations related to the course, student conduct, and library.
3. I will not be involved in any sort of political, ethnic, and sectarian activities; neither will I organize nor become a member of any political student party/organization or any student grouping.
4. I undertake that if I am found/ involved in any unlawful activity or providing incorrect information at any stage, the College shall have the right to cancel my admission without assigning any reason.
5. Furthermore, if I have forged, tampered with documents, and given a false statement or suppressed information about academic qualification to support this selection. I shall have no objection to expulsion or rustication from the college without assigning any reason.
6. I will not object if the Security deposit is forfeited due to termination from the course on disciplinary grounds OR discontinuation due to personal reasons.
7. In case of expulsion/ termination from the program on the basis of gross misconduct/violence /terrorism, I will not challenge the decision in any court of law in Pakistan.
8. I understand that any certificate and/or testimonials shall not be provided by this college if I decide to leave, without completing the training program.
9. In addition to the above, I further undertake that I shall not claim refund of the fee paid at the time of admission.
10. After taking admission in the college, if I want to leave the college, then I will be responsible for paying the complete dues from 1st semester to 8th semester (4 years) to the college.
11. I make a solemn promise that I shall be responsible for paying the college dues on the due dates and will not ask for any concession.
12. I accept that continuous absence for more than 15 days without prior information may lead to termination from the course by the authorities concerned.
13. Smoking and use of other drugs, alcohol, etc. are not allowed in college.
14. The Director of Sarhad Institute of Nursing and Health Sciences has the authority to rusticate me from the college if I violate the discipline/ rules and regulations of the college, and I will not have the right to challenge this in any court of law.
15. I will be responsible for my University, Hospital, HERA, Examination, PNMC, Hostel, Transport charges which are other than semester fee.
16. As per the rules of KMU Notification, Sarhad Institute of Nursing and Health Sciences, I will pay an annual 10% increase in semester fee
17. I will pay the Semester Fee within 15 days.
I FURTHER DECLARE THAT FAILURE TO COMPLY WITH THE ABOVE-MENTIONED CONDITIONS AT ANY STAGE, I WILL BE HELD BACK FOR ONE (01) ACADEMIC YEAR OR EVEN TERMINATED AS DECIDED BY THE DISCIPLINARY COMMITTEE. NO ONE FROM MY SIDE WILL CHALLENGE THE DECISION OF THE COLLEGE IN ANY COURT OF LAW
